We welcome new patients to Identity Dental Care with a friendly smile
Once you have booked your appointment with us, we aim care for your individual dental needs. We have a relaxing reception and our staff will offer you a drink while you complete your medical history questionnaire if you have not already completed and returned it. If you wish to complete your medical questionnaire prior to your appointment, please click here.
You will then be greeted by your dentist and introduced to your dental nurse. Your dentist will then guide you through our 15 point dental health check.
You will be able to discuss treatment options and when you are happy with your individual treatment plan, you will be given a copy.
Your first visit may take a little longer than you are used to as we take the time to get to know you and what you want from us.
Any of our staff will be happy to show you around the practice before your initial appointment.
We accept all major Credit & Debit cards including Amex at the practice.
15 point Dental Health Check - What you can expect from your visit to the practice
Questions that we will ask you at your visit and the information that we record and share with you for your individual dental care plan
- How healthy do you feel that your teeth and gums are and if you would like to change any aspect of your smile?
- Any problems that you may have in your mouth?
- We will ask about your medical history and medications that you take at each appointment as this could have an effect on the treatment that is prescribed.
- We will ask about Risk factors for gum disease like:
- Smoking
- Alcohol
- Stress
- High blood pressure
- Heart disease
- Diabetes
- Alzheimer's
- Osteoporosis
- Obesity
What we will look for and record:
- Progress checks on your x-rays and hygienist visit.
- We will check if you are happy with your teeth and gums to enable us to offer you individual dental care.
- We will check your jaw joints.
- We will check the outside of your mouth and neck.
- We will check the lining of your mouth for oral cancer, ulcers & mouth piercings.
- We will check your gum health.
- We will advise on areas where plaque may be found in your mouth and advise you on your oral hygiene routine and any products that may make your routine easier for you.
- We will check your teeth for wear like erosion (acidic diet or reflux), attrition and abfraction (clenching or grinding your teeth) or abrasion (heavy brushing). We will check if the root surfaces have been exposed by the gums shrinking and for any fractures.
- We will check any restorations like fillings and crowns that you may have, to make sure they are sound.
- We will check the areas under your restorations and in between the teeth with x-rays.
- We will check if your bone levels offer healthy support for your teeth.
